Reminder of Mandatory Vaccination for 6th Graders

State law requires all 6th grade students to have a TDAP vaccination prior to the start of the school year. Vaccinations are available free of charge at the Charlotte County Health Department every Monday from 8:30 until 11:00 a.m. or you may go to your health care provider if you prefer. The Charlotte County Health Department will also offer free TDAP shots for rising 6th graders on August 5, 2015 from 12:00- 6 p.m.  Parents will need to bring insurance information and fill out the required forms.

Parents are strongly encouraged to take advantage of these opportunities to get TDAP shots for rising 6th graders as these shots are required for school entrance. If rising 6th grade students have already gotten the shot and have proper documentation from a health care provider, it needs to be submitted to Central Middle School as soon as possible. Documentation may be submitted any weekday 8:00-4:00.   Please contact Central Middle School at 434-542-4536 with any questions.
